In this role, you will rotate between inpatient and outpatient kidney transplant, liver transplant and transplant surgery patients. PGY1 residency required. PGY2 in solid organ transplant, preferred. Monday - Friday Day 2 weekends per year. The pharmacy at Barnes-Jewish uses an integrated practice model with more unit-based positions. Here, department staff has access to a number of drug information resources, including many online references accessible from department computers. An on-site drug information center provides support for more in-depth questions. Preferred QualificationsRole PurposeEvaluates and recommends appropriate drug therapy to achieve optimal cost-effective patient outcomes, by working directly with physicians, other health professionals, and patients. Participates in the development, provision and evaluation of comprehensive pharmaceutical care and quality assurance programs through scholarly activities. Serves as a resource to the pharmacy staff and other hospital departments. ResponsibilitiesAssess the overall status of the patient's health condition and evaluates medication profile for optimal patient outcomes. Monitors therapy and goals of care.
